Employee Group Application
Who is Eligible to Join?
Those individuals eligible to join Baptist Hospital Credit Union meet at
least one of the following criteria:
- Any employee of the St. Thomas Health Services System and their affiliate hospitals or employee groups. This includes any employee, physician or organization under a contractual
relationship providing services to the St. Thomas Health Services System and its affiliates or partners.
- Any employee of a staff physician authorized to provide services for the St. Thomas Health Services System and its affiliates.
- An employee group with less than 3.000 employees
within a 25 mile radius of Baptist Hospital Credit Union who wish to have access to a credit union.
- Immediate family member of any of the above groups.
Starting Your Membership Account
To start your membership, you must open a Share/Savings account which will give you
ownership in the Credit Union.
To do so:
- Complete, print and bring the Online Membership Application to the Credit Union office.
Or, you may return the application by fax or mail with your signature notarized.
- Deposit a minimum of $50 or begin automatic deposits to build a $50 balance. Call us for the form or come by our office and complete.
- Bring your valid driver’s license to our office or send a copy by fax or mail that has been notarized as a “true and exact” copy.
Once the Share/Savings account is open, a member is eligible for all services
offered by the Credit Union.
Membership
accounts closed within 90 days of opening will be charged a $25.00 fee and those
closed twice or more will be charged a $25.00 fee.
Once a Member, Always a Member!
The charter provides that a member of the Baptist Hospital Credit Union is a
member as long as $50 is in the individual’s Share/Savings account.
Should you change jobs, move away from the area, or retire you can still
maintain your membership.
